You can measure almost anything now. Genetics. Microbiome composition. Environmental toxins. Hundreds of biomarkers from a single blood draw.
The problem isn't access to data anymore. It's knowing what to do with it.
Most people I work with aren't lacking information. They're drowning in it without a framework to make sense of what matters. They've had labs done, sometimes very advanced ones. They've been told things look "normal," or handed a list of abnormalities with no real plan attached. Some feel off. Some feel fine. Most land somewhere in between.
That's usually where this work begins.
Start With Function
Before going deeper, I want to understand how your body is actually performing. It’s not just what your labs say, but how your systems are working together.
That means starting with your story and taking stock of what’s changed (see previous newsletter on Feeling ‘Off’). In short, it’s looking at things like:
Fitness — how your workouts feel compared to before
Sleep quality and recovery — how you feel upon waking, plus heart rate variability and resting heart rate trends, if you track these
Mood, focus, and energy throughout the day
These are global indicators. They often tell us more about your long-term trajectory than any isolated biomarker. Detailed testing matters, but starting with fragmentation rarely leads to clarity.
Go Deeper When It Changes Decisions
Once we have a baseline, we can layer on more advanced testing, selectively.
I'm looking for tests that do at least one of three things:
Clarify an unclear signal: why is this nonspecific inflammation marker high?
Change a recommendation: we should block cholesterol absorption from the gut first rather than production in the liver because you are a hyper-absorber.
Help track progress over time: is our intervention working?
That's where tools like advanced biomarker panels or metabolomic profiling can earn their place. But data should always be interpreted in your context.
Tests That Actually Matter
When I do order specific labs, there's a core set I return to consistently. They're well-validated, actionable, and frequently underused.
ApoB — A more precise measure of cardiovascular risk than standard LDL. Every atherogenic particle has one ApoB molecule, so it tells you how many of those particles are circulating—which matters more than cholesterol weight alone. This is one I consider nearly universal.
Lp(a) — Largely genetic, largely fixed, and largely ignored in standard panels. But elevated Lp(a) meaningfully increases cardiovascular risk independent of everything else. You need to know your number—once is usually enough—because it changes how aggressively we approach everything downstream.
Fasting Insulin (with glucose and HbA1c) — Insulin resistance can be present for a decade before glucose becomes abnormal. This is the earliest reliable window into metabolic dysfunction. Under-ordered almost everywhere.
hs-CRP — A systemic inflammation marker where the “high sensitivity” form is used to detect smaller increases associated with cardiovascular disease. It’s less useful as a standalone number and more useful as a chronically high signal that something is worth investigating.
Homocysteine — An amino acid whose elevated levels are associated with cardiovascular and cognitive risk. It’s frequently elevated, frequently missed, and often correctable.
Body composition — Not a lab, but essential. Scale weight is nearly meaningless. Lean mass, visceral fat, and bone density all have direct implications for metabolic health and longevity. Get a DEXA scan (available direct-to-consumer, minimal radiation) or do a bioimpedance scan with a smart scale at home or the gym (less accurate, no radiation). For a visceral fat risk estimate alone, measure your waist circumference.
Hormone panels — Testosterone (Total, Free, SHBG), estrogen, FSH, LH, full thyroid (TSH, free T3, free T4), and consider cortisol patterns for chronic stress. These often explain what standard labs miss in people who feel off but test "normal."
Ferritin — The best early measure of iron stores, often flagging deficiency long before anemia shows up on a standard CBC. Frequently low in menstruating women, endurance athletes, and plant-forward eaters, it’s also one of the most common, most correctable drivers of fatigue and poor exercise tolerance I see. It's also an inflammatory marker, so a high number needs context rather than reassurance.
Omega-3 Index — Measures the essential fatty acids, EPA and DHA, as a percentage of your red blood cell membranes, which is a stable read on long-term omega-3 status, not just what you ate yesterday. Most people land well below the protective range for heart and brain health. It's directly modifiable through diet and supplementation, and you can retest to confirm you've actually moved it.
Vitamin D — Insufficiency is widespread and straightforward to correct, but response to supplementation varies enough that guessing isn't good enough. Relevant to bone health, immune function, and more. One of the few deficiencies where measure, treat, and re-measure is genuinely clean. Despite the inconsistent evidence on the benefits, I consider it a low-hanging insurance policy. Aim for 40-60 ng/dL.
Vitamin B12 — Essential for nerve function, red blood cell production, and cognition. Deficiency is common in older adults, plant-based eaters, and anyone on metformin or long-term acid reducers. Standard reference ranges run low enough that functional deficiency often hides inside "normal."
Standard metabolic panel + CBC — The foundation. Liver, kidney, electrolytes, blood counts. Not glamorous. Non-negotiable.
The through-line: these tests are ordered because the results reliably inform decisions. Not because they're comprehensive but because they're useful.
Where I'm Cautious
Some of the most talked-about tests right now are promising but not yet consistently useful in clinical practice.
Genetic testing — Valuable in the right context (drug metabolism, some cancer risk, some cardiac risk), but most results are static and frequently over-interpreted without ongoing updates as the science evolves. And it’s often better to just measure the phenotype, the actual clinical reality, rather than guessing about genetic probabilities.
Transcriptomics — What your genes are actively doing. Fascinating from a research standpoint. Not ready for routine clinical decision-making.
Microbiome testing — Widely used, often misunderstood. Most meaningful when paired with data on what those microbes are actually producing—postbiotics, metabolites.
Exposome / toxin testing — Only worth pursuing if there's a concrete plan to reduce or eliminate exposures. Without one, it tends to generate more anxiety than insight.
Biological age tests — Appealing because they collapse everything into a single number. But the major epigenetic clocks often disagree with one another, test-retest variability can be meaningful, and no intervention has yet been shown to move them in a way that reliably translates to better outcomes. Interesting to watch, great for marketing and unhealthy comparisons to your peers, but not yet something I'd act on.
Testing Is Not the Work
Testing can create the illusion of progress and control. But a test result, by itself, doesn't improve your health. What matters is what happens next.
Every test I order is tied to a clear interpretation, a specific plan, and a way to measure whether that plan is working. The goal isn't to know more. It's to make better decisions earlier and with more confidence.
A Simple Way to Think About It
Start broad. Then go deep. Prioritize function over fragmentation. Use data to guide action. Reassess over time.
Health is dynamic and your care should be too.
